Instructions for completing on-line training:

1. Print the following:

   2. Psychiatric Assessment Practice Guideline TRUE FALSE QUIZ
   3. Training Survey Treatment of Depression
   4. Training Survey Psychiatric Evaluation


2. View the automated PowerPoint for each training.
   1. Depression Practice Guidelines
   2. General Adult Psychiatric Evaluation Practice Guidelines


3. Allow 2 hours for each training.


4. Complete the clinician attestation located at the bottom of each quiz.


5. Maintain a copy for your files (depending on your agency personnel training policies, you may want to furnish a copy to Human Resources Department).


6. To receive continuing education credit (2 hours for each training for GCBH RSN provider clinicians only), complete and send a copy of the True-False Quiz and the Training Survey (evaluation form) within 20 days to GCBH. Mail or Fax to:
